        Financial Information Sheet (FIS) - This Financial Information Sheet should be completed prior to a show cause hearing
        (a/k/a temporary orders hearing) or any trial in a Family Law or Divorce Matter in Harris County, Tx.  In Divorce and
        Family Law matters in the Harris County Family Courts require the parties are required to exchange completed FIS forms
        prior to the start of the hearing.  This form should be completed in its entirety, signed by the party, and returned to the
        Attorney at least one day prior to the hearing.  This will allow the Lawyer to review the form for final changes.

        Bureau of Vital Statistics Form (BVS) - This BVS Form must be submitted to the Harris County District Court in which your
        Family Law and/or Divorce Law matter is assigned, and it is usually submitted along with a final Order or Divorce Decree
        upon prove up and/or entry of said Order. Generally, the Harris County Family Courts require the information on this
        form before you will be allowed to go forward with final entry or prove up of your final Family Law Order or Divorce
        Decree Order.  This form should be completed and returned to the Attorney at least one day prior to the hearing on or
        prove up of the final Order.  This will allow the Lawyer to review the form for final changes.

        Child Support Information Sheet (FOCUS Form) - Like the BVS form, in some cases the FOCUS form must be submitted
        along with a final Family Law Order and/or Divorce Decree upon prove up and/or entry of said Order.  Generally, the
        Harris County Family Courts require the information on this form before you will be allowed to go forward with final
        entry or prove up of the Family Law Order or Divorce Decree.  This form should be completed and returned to the
        Attorney at least one day prior to the hearing on or prove up of the final Order.

        Request for Notice to Employer of Income Withholding (Wage Withholding Order) - This form is used to direct the Harris
        County District's Clerk's office to submit a family law wage withholding order which has already been signed by the Court
        to the employer of the Obligor (person obligated to pay child support).

        Customer Order Form for Copies - This is the Harris County form used to order copies of case file documents (such as
        Divorce        Decrees, Child Support Orders, Parentage Orders, Adoption Orders, or other similar family law orders) on file
        with the Harris Country District Clerk's office.

        Texas Child Support Disbursement Unit Direct Deposit Authorization Form - This form can be used by the Obligee
        (the person entitled to receive child support under a Family Law Order or a Divorce Decree or some similar Order) to
        establish a direct deposit of child support to a personal bank account.

        Information Required by Texas Family Code Section 105.006 - This form allows parties in family law matters in Harris
        County, Tx to notify each other and the Court when information required by the Texas Family Codes changes for a party. 
        A description of when this form is required, and under what circumstances it is used, is included on the form.
